Chapter 49: Crohn's disease and genetics. This is a specific training for the Inflammatory Bowel Disease Sensor. This is where you find it here in the Gastrointestinal Sensor package, the IBD Sensor. It helps you in early diagnosis and treatment and less in prevention. So, if you have already developed it and you have severe problems, it might help you in early diagnosis. When you have the disease, it will help you improve treatment.


Section 2: The disease

Now, Crohn's disease is a difficult topic. It is not very well understood yet. We do know that there are genetic factors that significantly increase your risk of developing the disease. So, we know about those and we can do a genetic test for them. We also know that smoking, especially in combination with genetic variations, significantly increases the risk. Some studies hint that excessive animal protein might be also a risk factor that increases the risk. It might also be that certain bacteria in the guts cause this problem. There is also the too hygienic hypothesis that has been brought up in a too hygienic environment, compared to our ancestors who were living outdoors. This might be one cause of Inflammatory Bowel Disease. For now, we know that genetic predisposition, damaged genes that you inherited, as well as smoking are specific factors. When a person has symptoms and goes to a doctor and he gets the right diagnosis of Crohn's disease and when the first symptoms a rise, then it is usually possible to have treatment with medication as well as a modified diet. This will often help people not to undergo surgery where the damaged parts of the intestine are surgically removed. So, if early detection is possible in this case, it often improves the outcome without the need for surgery. If you have symptoms for one year before you get the right diagnosis, then in most cases surgery is already necessary. Early detection is very important for people with this disease. These are the genes that cause an increased risk. This is actually cut out from the report. This is the NOD2 gene and we analyze three different genetic variations. Basically, 1% of the population has the T/T genotype at 2.52 fold risk, almost 3 times higher risk of developing the disease. Combined with this genetic variation, there is 3.9 fold increased risk and the following is actually the toughest variation; if you have a C/C genotype, you have a15 fold increased risk. This is the science behind it in case you are interested.


Section 3: The test

So, we do the genetic test. We find out you have the normal risk or an extremely increased risk. If you have an increased risk, the first part helps you with early detection. In this program, we tell you what the symptoms are. You will detect the symptoms immediately when they develop and it will help you go to a doctor and get immediate treatment. This is the first step. The second part is better treatment. There is the Pharmacogenetics training, the Pharmaco Sensor, that I would recommend. This is part of this. We do test the genes that are breaking down certain drugs and then we can tell you that this drug is likely to be broken down too slow, so you need less of it. It might cause side effects or it might not work at all.


Section 4: Early detection

Let us look at early detection. First, you should pay attention to the symptoms. In the report, you will find out what the symptoms are. It is very easy. It tells you definitely not to smoke if you have an increased risk. You should eat less animal protein because it is likely linked. By following these, you will be able to slightly reduce the likelihood of the disease but more importantly it will help you diagnose it in time and get immediate treatment. In addition, Crohn's disease usually comes in phases. You have a phase of a lot of problems and then a phase where you feel better again. We can help you modify your diet to eat certain types of food during the more intense phases to help you combat the symptoms better. This is what the first stage in the program will help you with. It is about early detection and the best possible prevention and then better treatment.


Section 5: Drugs and Enzymes

Please do watch the pharmacogenetics training of the Pharmaco sensor where I explain that certain drugs are metabolized by certain enzymes. If this drug is used for Crohn's disease and the enzyme is not breaking it down and I take this drug regularly, then the amount of the drug is increasing constantly and then causes problems. Through testing the genes, we can then create a list of drugs. Here is a normal result as this drug is broken down normally. The dose is normal, but it might not be broken down at all. Then, you should use an alternative drug or you will see the recommended daily dosage modified. This will help you optimize your drug treatment and combat the disease more effectively. Really, this is more focused on early detection and better treatment, less on prevention because we just do not know scientifically how to prevent the disease most effectively.
